Submit the form with:
- An individual “headshot” photo – no larger than 2” x 2” of EACH person 6 years and over for each person being registered.
>> Names need to be on the backs of EACH picture <<
- Identification for each adult registering in the form of…
- A driver’s license for each adult bearing the Valleybrook community address or
- A lease (if you are a tenant) bearing the names of each person being registered.
- Identification for children 6 years of age or over…
- A school report card or school ID from Loring Fleming, Glen Landing, or Highland.
- A $2.00 per person check – made payable to the VBHOA for each person age 6 and above being registered.
